What does homebound mean?  Homebound means that leaving your home requires considerable effort or is challenging due to health issues.  If this describes you, reach out for in person home primary care and sick visits.

Meet Ashley, our family nurse practitioner with 20 years' experience in the nursing field.  Ashley treats patients with respect, dignity, and works with each patient keeping their goals in mind.

Must be paid up front. Virtual visits are $100 and can be done from anywhere. You can login from your smartphone, computer, or tablet.

 30 mins | Varies depending on Insurance

 If we are credentialed with your insurance provider, we can bill your insurance.  You would only be responsible for your co-pay, coinsurance, or deductible.

 In-Person Private Pay Appointment

 30 mins | $125

This is an option if you are not considered homebound and still wish to have an in-person appointment.   The in-person appointment is cash pay at $125 and must be paid up front.
