In order for us to be able to see you in person, you must be considered "homebound".  What does homebound mean?  Homebound means that leaving your home requires considerable effort or is challenging due to health issues.  If this describes you, reach out for in person home primary care and sick visits.

We are proud to be a part of the local community and are committed to giving back. The founder of Mosaic Healthcare was born and raised in Howard County, Indiana.  We work with local organizations to provide health education and resources to those in need.

Meet Ashley, our family nurse practitioner with 20 years' experience in the nursing field.  Ashley treats patients with respect, dignity, and works with each patient keeping their goals in mind.
